CRWG members,

	Attached is the simplified and corrected 2001 monitoring program that we
established at our 11/29/00 New Orleans meeting. I believe the table is self
explanatory. It is important to emphasize to the individuals that you give
this to that it covers ALL phases as revised, except phase 4  (see note
under Measurement Description). It is also important to point out that the
explanation of the use of the words "YES" and "NO" is contained in the notes
at the end. Hopefully this will be helpful to all.
